Long read. And a bit misleading. The trick is that unlike of civilian aviation, air force constantly lives very close to violation of flying rules. At first you think - o, they just took everything from pilot's record to blame him.
Only after you read the report carefully you begin to understand flaws in the chain of command, flaws in the leadership that slowly spoiled the pilot.
